Doctor-to-Patient Communication Templates

April 27, 2006

Dear Doctor:

As primary eye care providers, it is our responsibility to educate our patients and provide guidance when there is a cause for concern. The increased incidence of Fusarium keratitis is such a case.

Since health officials from the federal government are investigating whether Fusarium keratitis is linked to the ReNu® with MoistureLoc® brand contact lens solution, the American Optometric Association recommends you inform your contact lens patients to discontinue use of the Renu® with MoistureLoc® product immediately and find a substitute solution.

AOA introduces three resources to assist in informing your patient base:

  1. letter to patients describing Fusarium situation
  2. template for postcard communication
  3. phone script

These resources are designed to be personalized with the specifics of your office and may be printed on your practice letterhead.

Thank you for the excellent care you provide to your patients.

Sincerely,

Richard L. Wallingford, Jr., O.D.
