Air Force members who are required to sit for a paraoptometric examination must submit a copy of their certificate from Sheppard AFB optometry program in order to take the CPOA or CPOT examination unless they are currently certified as a CPO and are applying for the CPOA examination Duplicate certificates may be requested by submitting the student request form. Forms will be processed by Sheppard AFB personnel. Please note that processing may take 4 to 6 weeks.
