Complete your application using your name exactly as it appears on the photo ID you will present to PSI/LaserGrade on the examination date. Registering for the examination with a name that does not match the presented photo ID may result in denial of admittance at your scheduled examination appointment at PSI/LaserGrade and forfeiture of the entire registration fee.

ATTENTION DIALUP CONNECTION USERS: The application can take up to 5-6 minutes to be uploaded via email when using a slower dialup internet service. If you submit your application over a dialup connection you must contact the CPC office to verify receipt of your application.

This application is designed to work best with Adobe® Reader® 9. It is recommended that you update to the latest version before filling out your application.
